Monday through Friday - 9am -12:00pm, 1:30-5:00pm
At other times during the weekday, you may leave us detailed information on our voice mail system. Just follow the prompts. Appointments are scheduled calling 636-7000 and speaking with our staff or requesting the appointment through our voice mail system. Please give us detailed information. See below for more information on how to reach the doctor after hours.

Parking

Besides the parking spaces around our building, parking is also available on 9th street. Handicapped parking is in front of the office entrance.

Medications and Prescription Refills

We generally want you to refill your prescriptions at the time of your doctor's visit. If you take more than one or two medicines, it is a good idea to bring all of your medicines in a bag with each visit. That way we can verify that your medicines are being taken correctly. We will accept phone calls for refills in some instances but only during the times the office is open. Please don't call after business hours for refills. Once or twice a day we will call the various pharmacies and refill the requests we have received up to that point. Please don't call us to see if the prescription has been called in. Call the pharmacist if you need to. It is important that you plan ahead so no delays occur in taking your regularly scheduled medication. Drop by your refill to the pharmacist at least 24 hours in advance. Please note: for any phone call for a medication refill after 3pm on any weekday, we will not refill those medications until the following day.

After Hours Prescriptions - Plan Ahead

After hours, on holidays and weekends, your pharmacist is able, by state law, to fill your prescriptions in up to 3 day amounts (this is for standing prescriptions you've been filling for awhile). They will then call us the next business day for the refill verification. Please only do this as a last alternative. REMEMBER, don't get in a fix, PLAN AHEAD!

After hours/ 24 Hour Coverage

The physicians are available 24 hours a day, 7 days a week for emergencies. The "on-call" physician (either Dr.Bradd or Dr. Bernard [and sometimes, Dr. Wenzinger]) can be reached by calling the hospital at 636-0300 after regular business hours. The appropriate "on-call" physician will then be paged by the hospital operator and he will return the phone call. If it is a medical emergency, do not call the hospital to have the doctor paged but call "911". Please don't call any of the doctors at their home numbers since they may not be home and this will be wasted effort on your part. Remember, the hospital operator generally knows how best to get in touch with the on-call doctor.

Hospital Information

We admit our patients to Warren Memorial Hospital, just across the street. However, there are times when we may transfer our patients to the appropriate hospital for your best care.

Billing and Insurance Information

Unless we participate with a particular insurance carrier, medical services that are provided are expected to be paid for at the time of service. By upholding your responsibility for paying for services, we can hold the costs down and spend more time on medical care and not trying to collect debts due to us. We accept cash, personal checks, MasterCard, and Visa credit cards as appropriate forms of payment.

Insurance payments vary with the type of policy and insurance carrier. Check with your carrier if you have questions regarding your coverage. However, the charges resulting from the delivery of medical care are your responsibility. As a service to you, insurance claim forms will be filed for you (if needed). However, your medical bill is ultimately your responsibility. If you have questions, please refer to our credit policy which you signed when registering (or is posted in many areas of the office). If you are having problems paying your bill, tell us so we can assist you.

If you are covered by more than one insurance company, please let us know which company is primary (to be filed first and is responsible for payment first), and which is secondary (to be filed after the primary has paid). We cannot be responsible for knowing which insurance would be filed first.

Please advise our office of any change in your address, phone number, marital status, insurance information, etc. that have occurred since your last visit .

Any additional information requested by the insurance company to update their files, is YOUR responsibility. If payment of the claim is held up because of information you need to supply to your carrier, we will process this account and deal with the outstanding balance accordingly. Accounts are routinely turned over to an attorney or a collection agency when payment has not been paid within a reasonable amount of time.

Our office will be glad to complete other forms for you. Depending on the situation (such as complexity and urgency), we may charge you an additional fee for filling out forms not directly related to insurance filing. This charge is payable when the form is brought in. They cannot be done the day the form is brought in, so please be sure to see that we have the form as soon as possible.
